Many adults over 40 find that the approach that worked at 30 — eat less, move more — stops working. Weight settles around the middle, energy dips in the afternoon, and diets that once delivered now do little.

There are three shifts in nitric oxide, thyroid function and androgens that change the rules after 40. This article is the other half: the twelve-week sequence I actually run people through, what to measure while you do it, and how to tell it is working when the scale says otherwise.

Why the order matters

Symptoms come last. The body does not jump from healthy to struggling overnight — it moves through three phases.

First come silent changes at the cellular level: mitochondria slow down, oxidative stress rises, toxins accumulate in fat tissue. Nothing is felt. Then the body's systems begin to respond — hormones shift, insulin becomes less effective, the gut lining weakens — and blood work starts drifting without anything looking alarming on its own. Only in the third phase does anyone actually feel it: stubborn midsection weight, the 3pm crash, waking between 2 and 4am, brain fog, slower recovery.

Most weight-loss advice starts at the symptom end and ignores the two phases underneath. In thirty years of practice this is the single most common frustration people bring to me. Midlife weight gain is a signal, not a failure of discipline.

Each of these shifts feeds the others, so the order of repair matters. Make it safe for the body to release fat, restore cellular energy, rebalance the hormones, then reinforce until the new baseline holds. Detoxify, energise, rebalance, reinforce — four layers, twelve weeks.

The fat the scale cannot see

Before the protocol itself, the single most useful thing I can tell you about measurement.

Not all fat is equal. Visceral fat — the deep fat surrounding the organs — is metabolically active, releasing inflammatory cytokines such as IL-6 and TNF-alpha, plus free fatty acids, directly into the circulation feeding the liver. When it overflows, fat spills into organs never designed to store it — liver, pancreas, heart, muscle — as ectopic fat. I have written about this in detail in Visceral and Ectopic Fat.

Here is the part that matters for anyone following a twelve-week programme: when metabolic health improves, the body clears visceral and ectopic fat first — before the subcutaneous fat anyone can see.

The scale can sit still for months while your metabolic health genuinely improves. People give up at week five because the number has not moved, when the work underneath is doing exactly what it should. That is why the waist tape, not the scale, is the honest measure.

The daily foundations

These run every day, through all twelve weeks and beyond.

  • Ultimate 4 — one scoop daily, supporting healthy circulation, energy and metabolic function.

The four layers

Layer 1 — Detoxify (weeks 1–2)

Make it safe for the body to release fat. Start Ultimate 4 daily. Strip out alcohol, seed oils and ultra-processed foods. Add cruciferous vegetables. Walk daily.

This layer is not glamorous and people want to skip it. Don't. When circulation and antioxidant capacity are low, the body is poorly equipped to process what is released when fat stores break down — so it favours holding on to them.

Layer 2 — Energise (weeks 2–4)

Restore the cellular energy midlife has eroded. Add BOOST each morning and get 10 minutes of morning sunlight. Begin two to three strength sessions weekly — muscle is the metabolic engine you are rebuilding. Add one to two longer, easy-paced Zone 2 cardio sessions weekly.

Research reviews estimate adults lose in the order of 3–8% of muscle mass per decade after 30, with the rate accelerating after 60. Muscle is the tissue burning most of your resting calories. Every week you are not training, the thermostat drops a little further.

Layer 3 — Rebalance (weeks 4–8)

There is a loop worth understanding here. Visceral fat contains aromatase, an enzyme that converts testosterone into oestrogen. The more midsection fat a person carries, the faster they lose the very hormone that maintains muscle and promotes fat breakdown. Excess weight lowers androgens, and lower androgens make weight easier to gain — a self-reinforcing loop that "just try harder" cannot break.

Layer 4 — Reinforce (weeks 8–12 and beyond)

Keep every foundation running and keep training. Most resets fail here, because people stop. Retest and review all markers at week 12.

Track your progress

You cannot manage what you do not measure, and the scale is the wrong instrument.

  • Waist measurement, fortnightly — a better visceral-fat proxy than the scale.
  • Nitric oxide saliva test strip, weekly — aiming for the optimal range. Test strips take about 15 seconds.
  • Energy score 1–10 each afternoon; sleep quality 1–10 each morning.
  • Strength log — loads and recovery week to week, with a full review at weeks 6 and 12.

Diet and lifestyle — the four pillars

Supplements work with your biology; the four pillars build it. These are the targeted moves for this protocol, drawn from the framework in my book Happy Healthy YOU.

Diet. Build a plant-predominant plate — 30 different plants a week, leaning on greens, legumes, berries, turmeric and cinnamon. Hit your protein target at every meal. Eat light and early in the evening; an early dinner is gentle intermittent fasting that suits most adults over 40. Before eating, ask the four-pillars question: is this food building health or destroying health?

Movement. Take a 10–15 minute walk after meals — muscles take up glucose best right after eating. Keep two to three strength sessions weekly. Move every hour and hold a 5,000-step daily floor.

Sleep. Short sleep drives hunger hormones: appetite climbs and cravings follow a poor night. Protect seven to nine hours. No sugar after 6pm, no caffeine after midday — both fragment the deep sleep that regulates appetite.

Mindfulness. Daily breathwork, morning and night — chronically high cortisol signals the body to store fat around the midsection. Eat mindfully in a calm environment: slow down, chew well, notice fullness. This is attention, not restriction. And make movement social — the pillar you enjoy is the one you keep.
